Carry out strengths-based assessments under the Care Act 2014, empowering individuals to make informed choices and maintain independence.

Manage a varied caseload, including older adults, individuals with learning disabilities, mental health needs, and complex social care requirements.

Work closely with health professionals, carers, families, and voluntary sector partners to design and deliver integrated, outcome-focused support plans.
