Manage a caseload involving older adults, individuals with disabilities, mental health challenges, and those requiring safeguarding interventions.

Carry out complex assessments and care planning under the Care Act 2014 and Mental Capacity Act 2005, promoting independence and personalised outcomes.

Support the development of best practice by mentoring and supervising junior staff, contributing to a culture of learning, reflection, and continuous improvement.

Lead on multi-agency work to ensure integrated, effective, and outcome-focused services, particularly in cases involving safeguarding and hospital discharges.
