Job details: Experienced Occupational Therapist required to join a dynamic Reablement Service supporting residents to regain independence, confidence, and functional ability within their own homes.
- Provide high-quality Occupational Therapy assessments and interventions for individuals with complex care and therapy needs
- Complete holistic, strengths-based assessments to identify needs, risks, and opportunities for independence
- Develop and implement tailored reablement plans focused on maximising independence and functional ability
- Work collaboratively with reablement support workers, social care colleagues, and health partners to achieve improved outcomes
- Provide specialist clinical advice, equipment recommendations, and moving & handling guidance
- Support safe and timely hospital discharges and prevent avoidable admissions
- Undertake assessments of care needs for a cohort in receipt of long-term double handed care as part of a time-limited project
Working hours: 36 hours per week, Monday to Friday, 09.00 – 17.00.
