About the Role
We are seeking a skilled Band 6 Occupational Therapist with experience in stroke rehabilitation to join the community team at Longhill Health Centre. This locum role is initially for 3 months, supporting patients across Hull and the East Riding through tailored stroke rehabilitation interventions in their homes and community settings.
Main Responsibilities
- Deliver specialist occupational therapy assessments and rehabilitation programmes for stroke patients.
- Work autonomously across Hull and East Riding, managing a varied community caseload.
- Support individuals in regaining functional independence through evidence-based treatment plans.
- Use clinical reasoning and outcome measures to guide effective rehabilitation.
- Collaborate with MDT colleagues to ensure coordinated, patient-centred care.
- Maintain accurate and timely clinical documentation in line with HCPC and service standards.
Requirements
- Degree in Occupational Therapy (Post-grad qualification and 3+ years’ experience desirable but not essential).
- Experience in community-based stroke rehabilitation.
- Strong assessment, clinical reasoning and caseload management skills.
- Car driver with access to own vehicle and willing to travel across Hull and East Riding.
- SystmOne experience desirable but not essential.
- Ability to work independently and reliably in a community setting.
Working Hours
- Monday to Friday, 08:00 – 16:00.
