Undertake Care Act 2014 assessments on behalf of the London Borough of Brent Council, focusing on promoting independence, wellbeing, and person-centred care for adults with varying needs.

Manage a complex caseload including older adults, people with learning or physical disabilities, mental health needs, and those requiring safeguarding interventions.

Develop personalised care and support plans, ensuring outcomes are strengths-based, culturally sensitive, and co-produced with service users and carers.

Carry out Section 42 safeguarding enquiries, working closely with partner agencies to investigate concerns and protect adults from abuse or neglect.

Maintain accurate and timely case records and reports, ensuring compliance with statutory responsibilities, council policies, and professional standards.

Contribute to team learning and service improvement, engaging in supervision, reflective practice, and professional development within Brent’s Adult Social Care service.
