Undertake strengths-based assessments in line with the Care Act 2014, on behalf of Darlington Borough Council, to identify eligible needs and promote independence, wellbeing, and personal choice.

Manage a caseload of adults with varying levels of need, including older people, individuals with physical or learning disabilities, and those experiencing mental health issues.

Develop person-centred care and support plans, working closely with service users, carers, families, and partner agencies to ensure effective, coordinated support.

Safeguard vulnerable adults by recognising and responding to abuse, neglect, or exploitation, and participating in Section 42 Enquiries as appropriate.

Maintain accurate and timely records in line with council policies and legal frameworks, including the preparation of reports for case reviews, best interest meetings, and court proceedings.

Contribute to service development by engaging in supervision, reflective practice, training, and team meetings, promoting continuous learning and quality improvement within Adult Social Care.
