Deliver statutory adult social care duties for The Borough Council of Gateshead, completing Care Act 2014 assessments and reviews to identify and meet eligible needs.

Manage a varied caseload including older adults, individuals with physical disabilities, learning disabilities, and mental health conditions, promoting independence and wellbeing.

Develop personalised care and support plans in collaboration with service users and carers, ensuring strengths-based, outcome-focused approaches.

Undertake safeguarding enquiries under Section 42 of the Care Act, assessing risk and coordinating multi-agency protection plans for adults at risk.

Carry out Mental Capacity Act assessments and contribute to best interest decisions where individuals lack capacity, in line with statutory guidance.

Maintain accurate and timely case records and participate in supervision, training, and reflective practice to uphold high standards within Gateshead’s Adult Social Care services.
