- Join Gateshead Council to deliver high-quality statutory social work services for adults, promoting independence, dignity, and wellbeing across the community.
- Conduct Care Act assessments, safeguarding enquiries, and mental capacity evaluations to identify needs and risks, ensuring appropriate support and protection.
- Create and review personalised care and support plans, working collaboratively with individuals, families, and carers to achieve positive outcomes.
- Work in partnership with health professionals, housing services, and voluntary organisations to coordinate care and improve service delivery.
- Maintain accurate and timely case records, ensuring compliance with relevant legislation including the Care Act 2014 and Mental Capacity Act 2005.
- Engage in supervision, reflective practice, and ongoing professional development, contributing to continuous improvement and maintaining high standards of care.
