Take the lead on complex and high-risk adult social care cases for Derbyshire County Council, including safeguarding, hospital discharges, mental health, and continuing healthcare.

Provide professional leadership, guidance, and support to social workers and other team members, promoting reflective practice and high standards of care.

Undertake advanced assessments and develop person-centred support plans in line with the Care Act 2014, ensuring a strengths-based, outcomes-focused approach.

Lead on Section 42 safeguarding enquiries, chair strategy discussions where appropriate, and work with multi-agency partners to manage risk effectively.

Apply expert knowledge of the Mental Capacity Act 2005, completing complex assessments and contributing to best interest decisions and court applications as necessary.

Contribute to service development, policy implementation, and quality assurance initiatives, supporting innovation and continuous improvement in adult social care.
