Job Overview
Information and Assessment Officer required to support Adult Social Care services, delivering high-quality, customer-focused information, advice, and initial conversations to Hackney residents and their carers.
Key Responsibilities
Work with Social Workers and other professionals within Adult Social Care to provide effective, accessible information and Initial Conversations.
Deliver a high standard of customer care, offering advice and assistance on council services and services provided by statutory, independent, and voluntary agencies.
Carry out initial conversations using a strengths-based approach to identify needs and promote independence.
Act as a Trusted Assessor, providing simple equipment to older and disabled people to help them maintain independence.
Produce individually tailored plans for simple equipment provision.
Recommend and requisition Telecare services where appropriate.
Support the setup of social care services following assessment.
