Job Details
An Information and Assessment Officer is required to support Adult Social Care services, working closely with Social Workers and other professionals to deliver effective, customer-focused support to Hackney residents and their carers. JD available on request.
Work alongside Social Workers and other Adult Social Care professionals to provide accessible, high-quality information and initial conversations.
Deliver a high standard of customer care through the provision of information, advice and assistance regarding council services and support available from statutory, independent and voluntary agencies.
Undertake initial conversations using a strengths-based approach to identify needs and promote independence.
Act as a Trusted Assessor, providing simple equipment to older and disabled people to help them maintain independence.
Produce individually tailored plans for simple equipment provision.
Recommend and requisition Telecare solutions where appropriate.
Support the setup of social care support in line with assessed needs.
