As a Speech and Language Therapist with Lancashire and South Cumbria NHS Foundation Trust, working in community settings, you will provide specialist assessment and intervention for adults with neurological conditions, dysphagia and rehabilitation needs. Operating at Band 6, you will collaborate closely with patients, families and multidisciplinary teams to deliver person-centred care and optimise communication, swallowing and functional outcomes.
Conduct comprehensive assessments of speech, language, communication and swallowing for adults with neurological and rehabilitation needs.
Develop, implement and evaluate personalised therapy programmes based on clinical priorities and best practice.
Work collaboratively with patients, families, carers and healthcare professionals to coordinate care and support.
Provide advice, strategies and support to improve communication, swallowing and functional independence.
Maintain accurate and detailed clinical records in accordance with professional and organisational standards.
Contribute to service development, quality improvement and multidisciplinary team initiatives within the speech and language therapy service.
