Occupational Therapist

Brief Job Overview

Tripod Partners are currently recruiting for an experienced Occupational Therapist to join Flintshire County Council at T? Draig, St David’s Park, Deeside, within North East Wales Social Services. This is an ongoing locum opportunity offering the chance to support individuals and carers across the community to overcome long-term barriers caused by health conditions and maximise independence.

The successful Occupational Therapist will provide person-centred, outcome-focused assessments within people’s homes and other community settings. You will work with individuals and their carers to identify barriers, develop action plans and build on existing strengths to optimise function and resilience.

The role includes specialist assessment and recommendations around manual handling, seating, equipment and minor and major adaptations. You will work closely with colleagues across health, social care, housing and other agencies to deliver practical solutions that enable people to remain safely and independently at home or within their community.

Key Responsibilities

  • Complete the full Occupational Therapy process across community settings, including people’s own homes, sheltered and residential accommodation and schools.
  • Manage a caseload of complex cases and work collaboratively with colleagues across housing, health, care services and the third sector.
  • Undertake outcome-focused Occupational Therapy and specialist assessments relating to manual handling, positive risk-taking, seating and the home environment.
  • Develop detailed recommendations and interventions to support individuals in achieving their occupational outcomes.
  • Work with individuals and carers to formulate action plans that remove barriers, build on strengths and optimise functional ability.
  • Provide advice, demonstrate techniques and share knowledge to help individuals achieve optimal levels of functioning and support carers where possible.
  • Assess for and provide disability equipment, including consideration of core stock and research into non-stock specialist equipment where required.
  • Recommend appropriate equipment and techniques to improve safety, independence and ability to complete activities of daily living.
  • Assess and recommend minor and major adaptations to people’s homes, working in conjunction with housing colleagues and partners.
  • Where adaptations are not appropriate, assist with the initiation and completion of re-housing assessments.
  • Review interventions and support individuals to develop ongoing resilience before cases are closed.
  • Work collaboratively with individuals, families and multiple agencies across health, social care, housing, third sector and private sector services.
  • Share appropriate information, undertake joint working and provide professional Occupational Therapy perspectives in medical or legal settings where required.
  • Maintain timely, accurate and high-quality paper and electronic records in accordance with professional, local governance and data protection requirements.
  • Maintain continuing professional development through appropriate training, clinical supervision and reflective practice.
  • Participate in student fieldwork education and maintain a professional portfolio in accordance with professional requirements.

About You

We are looking for an Occupational Therapist with strong community-based experience and the ability to work confidently with individuals and carers in their own homes. You should have a person-centred, enabling approach and be able to assess complex functional needs while identifying practical solutions that promote independence and safety.

Experience in minor and major adaptations, manual handling and community assessments is essential. You will also need excellent communication skills and the ability to work effectively across health, social care, housing and third-sector services.

Requirements

  • Degree in Occupational Therapy.
  • Current HCPC registration.
  • Understanding of the principles and responsibilities arising from relevant legislation, including the Social Services and Well-being (Wales) Act.
  • Experience providing Occupational Therapy services to individuals and carers within their own homes.
  • Experience working within community Occupational Therapy using a person-centred, enabling approach.
  • Experience assessing and recommending minor and major adaptations.
  • Experience undertaking manual handling assessments.
  • Experience completing community-based Occupational Therapy assessments.
  • Ability to undertake specialist assessments, including seating, manual handling, positive risk-taking and environmental assessments.
  • Strong knowledge of disability equipment and adaptations.
  • Excellent communication and multi-agency working skills.
  • Commitment to continuing professional development, reflective practice and professional standards.
  • Ability to undertake home visits across the county.
  • Access to own transport is required.
