Job details: Community Stroke Team. May be required to work in different locations within the Community Stroke Team across Lambeth and Southwark. Providing Speech & Language Therapy assessments and treatments in a community setting to a caseload of patients with a diagnosis of Stroke, working within an MDT on goal focussed rehabilitation. JD available on request:
- The post holder will provide a specialist Speech and language Therapy support to individuals as part of the Community Stroke Service and will work closely with other members of the multidisciplinary team (MDT) to ensure a coordinated approach to client care. They will independently and effectively manage a caseload of individuals diagnosed with a stroke who have complex needs. The post holder will be using evidence based, client-centred principles to assess, plan, implement and evaluate interventions in order to minimise the impact of stroke on the individual’s communication, swallowing and social integration.
- The post holder will be responsible for the supervision of rehabilitation assistants (RSWs) and junior SLT staff or SLT students on placement.
- The post holder is expected to achieve individual activity target (face to face contact) of at least three visits a day.
- Monthly supervision and annual appraisal (Personal Development Plan – PDP) will be provided by a Senior SLT. There is the opportunity for peer support and SLT team IST.
Working hours: Monday to Friday, 08.30 – 16.30.
