Senior Social Worker – Joint Emergency Team (JET)

We are recruiting an experienced and motivated Senior Social Worker to join the Joint Emergency Team (JET) at the London Borough of Lambeth. This dynamic service operates seven days a week, providing rapid, responsive support to individuals in crisis across the borough. The JET team’s primary goal is to help people remain safely in their homes, reducing unnecessary hospital admissions and ensuring timely community-based intervention.  
Key Responsibilities
- Deliver timely and effective crisis interventions to prevent hospital admissions and support safe discharge back into the community.
- Undertake holistic assessments of need, risk, and capacity, ensuring all decisions promote independence and wellbeing.
- Support and lead on Continuing Health Care (CHC) assessments for individuals in the borough without an allocated social care worker (training provided where necessary).
- Work closely with health professionals, community services, and families to ensure joined-up care and support.
- Develop care plans that are proportionate, strengths-based, and focused on enabling independence.
- Participate in the team rota, including one late shift per week (12:30pm – 8:30pm) and one weekend in four (enhanced payment provided for weekend shifts).
- Ensure timely and accurate recording of all casework in line with professional and statutory standards.
- Engage in supervision, reflective practice, and professional development activities to maintain high standards of care.

Requirements
- Qualified Social Worker registered with Social Work England.
- Experience of working in a fast-paced, front-line service such as hospital discharge, reablement, or crisis response.
- Knowledge of Continuing Health Care (CHC) processes (training provided for suitable candidates).
- Ability to work flexible hours, including one late shift per week and one weekend in four.
- Excellent communication, assessment, and decision-making skills.
- Ability to work autonomously while contributing positively to a multi-disciplinary team.
- Commitment to ongoing professional development and reflective practice.
