Manage complex and high-risk adult cases for Bradford Metropolitan District Council, including safeguarding, mental health, hospital discharge, and long-term care planning.
Provide professional support and informal supervision to social workers and students, promoting reflective practice and high-quality service delivery.
Undertake thorough, strengths-based assessments under the Care Act 2014, developing person-centred support plans that promote independence and wellbeing.
Lead on safeguarding enquiries under Section 42, coordinating with multi-agency partners to ensure effective protection and risk management.
Apply the Mental Capacity Act 2005 in practice, completing capacity assessments and contributing to best interest decisions and legal processes where necessary.
Support service development through participation in quality assurance activities, case audits, and continuous improvement initiatives within the adult social care team.
