Job Title: Senior Occupational Therapist – Band 6

Job details: Community Therapies – Intermediate Care (Falls, Frailty, Rehabilitation). JD available on request.
Undertake holistic OT assessment, clinical diagnosis and individualised treatment plans for patients with complex and/or chronic presentations, including palliative patients
Contribute to effective inter-disciplinary working, providing a high-quality day to day service to the active caseload
Provide supervision, support and line management to junior staff, and mentorship to students
Support case management by providing expert clinical advice and therapy to community-based patients at risk of deterioration or avoidable hospital admission
Work with senior staff to develop and enhance the service in partnership with Integrated Care Teams and relevant stakeholders
Participate in a rota within Intermediate Care covering 08:00–20:00, 7 days a week across service streams
