Job details:  Senior experienced Occupational Therapist required to join fast paced, dynamic, and evolving Reablement team.
The successful candidate will work closely with the team manager to ensure that the service promotes independence in line with the councils values and behaviours.
Skills/experience: qualified Occupational Therapist as well as being HCPC registered. The successful candidate will:
–       Provide supervision and case management to your supervisees and wider team.
–       Work closely with the enablement care provider to ensure that Reablement timeframes are adhered too.
–       Lead on complex cases and support workers to undertake effective functional assessments, Mental Capacity Assessments and make recommendations and set goals to enable function.
–       Ensure the team are completing assessment and addressing identified needs to promote well-being in accordance with the Care Act 2014.
–       Provide/recommend equipment and adaptations to support safety and independence.
–       Complete moving and handling assessments and plans
–       Provide advice and support to multidisciplinary colleagues.
–       Support the team manager with the day to day running of the team.
–       Act as duty manager, screening referrals and supporting the flow of discharge from the acute and community reablement referrals.
