Diabetes Specialist Nurse

Location: Southport
Contract: Full-time (Part-time considered)
Flexible Working: Compressed hours, evenings, and weekends considered
Pay Rate: £30.24 per hour
Duration: 3 Months

Role Overview:
We are seeking an experienced Diabetes Specialist Nurse to join the Diabetes team in Southport. This role will focus on delivering expert care, education, and support to patients with diabetes while working collaboratively within a multidisciplinary team. V300 prescribing qualification is essential.
Key Responsibilities:

  • Provide specialist diabetes care and management for patients in the community and clinical settings.
  • Prescribe and manage medications under the V300 prescribing qualification to support patients’ individual treatment plans.
  • Deliver comprehensive patient education, including insulin initiation, titration, and blood glucose monitoring.
  • Work closely with GPs, endocrinologists, dietitians, and other healthcare professionals to ensure coordinated patient care.
  • Assess, diagnose, and manage diabetes-related complications, providing proactive interventions.
  • Lead and contribute to diabetes clinics and outreach programmes to enhance patient outcomes.
  • Educate and support patients, families, and carers on self-management strategies.
  • Maintain accurate patient records and ensure compliance with NICE guidelines and best practices.
  • Participate in service development initiatives to improve diabetes care pathways.

Requirements:

  • Registered Nurse (NMC) with a V300 prescribing qualification (ESSENTIAL).
  • Significant experience in diabetes management and patient education.
  • Strong understanding of insulin therapy, blood glucose monitoring, and diabetes technology.
  • Excellent communication and leadership skills with the ability to work independently and as part of a team.
  • Willingness to work flexible hours, including evenings and weekends, if required.
