Qualified Social Worker – Adults (Older People)
Brief Job Overview
We are looking for an experienced and dedicated Qualified Social Worker to join the Adult Social Care Team at the London Borough of Merton. This role focuses on supporting older people within the community to maintain independence, dignity, and quality of life through high-quality, person-centred social work.
You will work collaboratively with individuals, carers, and partner agencies to assess needs, develop care and support plans, and ensure that older adults receive the right support to live safely and independently at home or in appropriate care settings.
Key Responsibilities
- Undertake Care Act 2014 assessments for older people to identify needs, strengths, and outcomes.
- Develop and implement person-centred care and support plans that promote independence, wellbeing, and dignity.
- Conduct reviews of existing care plans to ensure ongoing relevance and effectiveness.
- Apply the principles of the Mental Capacity Act 2005 and undertake Best Interest decisions where appropriate.
- Work with adults, their families, and carers to achieve positive outcomes and maintain independence for as long as possible.
- Collaborate with health, housing, and voluntary sector partners to coordinate integrated care and support.
- Respond to Safeguarding Adults concerns and carry out Section 42 enquiries when required.
- Provide professional advice and support to individuals and their families regarding community resources and preventative services.
- Ensure casework and decision-making align with relevant legislation, policies, and best practice standards.
- Maintain accurate, timely records and contribute to team meetings, supervision, and case discussions.
About You
You are an experienced social worker with a strong background in Adult Social Care, particularly in supporting older people. You bring empathy, professionalism, and excellent communication skills to your work, and you are committed to enabling independence, choice, and wellbeing. You thrive in a collaborative, multidisciplinary environment and have the ability to manage a varied caseload effectively.
Requirements
- Qualified Social Worker registered with Social Work England.
- Proven experience working within Adult Social Care, specifically with older people.
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults frameworks.
- Experience in completing assessments, care planning, and reviews for adults and older people.
- Excellent communication, negotiation, and report-writing skills.
- Ability to work collaboratively with multi-agency partners to deliver effective care and support solutions.
- Commitment to strengths-based, person-centred practice.
- Ability to prioritise and manage a complex caseload effectively.
- Knowledge of local community resources and services that support older people (desirable).
