Social Worker (Adults) – Learning Disabilities & Autism

Brief Job Overview

 
We are recruiting an experienced and dedicated Adults Social Worker to join the Learning Disability and Autism Team at Bracknell Forest Borough Council. This is a fantastic opportunity to work in a dynamic, person-centred service supporting adults with learning disabilities and autism to live fulfilling, independent lives within their community.  

This role focuses on carrying out Care Act assessments, developing care and support plans, and working collaboratively with families, carers, and professionals to promote independence, inclusion, and wellbeing. The position is hybrid, with the expectation to be in the office three days per week.

Key Responsibilities

 

  • Conduct Care Act 2014 assessments and identify strengths, needs, and desired outcomes for adults with learning disabilities and autism.
  • Develop and implement person-centred care and support plans that promote independence, choice, and control while remaining cost-effective and legally compliant.
  • Apply a strengths-based approach using the Conversations Model to help people connect with their community and natural support networks.
  • Work collaboratively with adults, carers, advocates, and partner agencies to achieve the best outcomes.
  • Undertake reviews and monitoring of care plans to ensure that services remain relevant and effective in meeting assessed needs.
  • Participate in duty arrangements within the team, responding to new referrals and urgent situations as required.
  • Work with family carers to assess their needs and develop tailored support plans in line with the Care Act 2014.
  • Ensure all casework and decision-making aligns with the Mental Capacity Act 2005, acting in an individual’s best interests when capacity is lacking.
  • Participate in safeguarding adults assessments and contribute to multi-agency safeguarding planning and reviews.
  • Attend and contribute to multi-disciplinary meetings, connecting individuals and families with their local community and support networks.
  • Prepare and present cases for approval in accordance with the council’s Scheme of Delegations.
  • Ensure all departmental policies and procedures are adhered to, maintaining accurate and high-quality case records.

About You

 
You are an empathetic, motivated, and experienced social worker with a passion for working with adults who have learning disabilities and autism. You bring a strong understanding of statutory frameworks and are confident in applying strengths-based, person-centred approaches. You are proactive, solution-focused, and committed to empowering individuals to live fulfilling lives within their communities.

Requirements

 

  • Qualified Social Worker registered with Social Work England.
  • Essential experience working with adults with learning disabilities and autism.
  • Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults procedures.
  • Experience completing assessments, support planning, and reviews within statutory adult social care.
  • Ability to work collaboratively with adults, carers, families, and multi-agency partners.
  • Excellent written and verbal communication skills with strong report writing ability.
  • Full UK driving licence and access to a car for community visits.
  • Ability to work in the office three days per week as part of a hybrid working model.
