Experienced Social Worker – Adults
Brief Job Overview
We are seeking an experienced and proactive Adults Social Worker to join Gloucestershire County Council on a 3-month locum contract. This is an exciting opportunity to join a supportive and forward-thinking Adult Social Care service dedicated to empowering individuals, promoting independence, and delivering high-quality, person-centred care across Gloucestershire.
As an experienced practitioner, you will undertake assessments, coordinate care planning, and carry out safeguarding work to ensure adults receive the right support to live safely and independently within their communities.
Key Responsibilities
- Undertake Care Act 2014 assessments to identify needs, strengths, and outcomes for adults requiring care and support.
- Develop and implement person-centred care and support plans that promote independence, wellbeing, and choice.
- Work collaboratively with individuals, carers, families, and partner agencies to ensure effective and coordinated service delivery.
- Carry out Safeguarding Adults enquiries under Section 42 of the Care Act, ensuring proportionate and timely intervention.
- Apply the Mental Capacity Act 2005 and make best interest decisions where appropriate.
- Undertake reviews of existing care plans, ensuring support remains appropriate and cost-effective.
- Work closely with health and housing partners to facilitate hospital discharges and prevent unnecessary admissions.
- Maintain accurate and up-to-date case records, reports, and documentation in line with professional and council standards.
- Promote a strengths-based, outcome-focused approach to practice across all aspects of work.
- Participate in regular supervision, team meetings, and professional development opportunities to maintain and enhance practice quality.

You are an experienced and confident social worker who is passionate about supporting adults to live fulfilling and independent lives. You have strong assessment and communication skills, a clear understanding of statutory responsibilities, and a collaborative approach to multi-agency working. You are committed to continuous improvement and reflective practice, always striving for positive outcomes for service users.
Requirements
- Qualified Social Worker registered with Social Work England.
- Extensive post-qualified experience in Adult Social Care.
- Sound understanding of the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults legislation.
- Experience working in complex case management, assessments, and reviews.
- Excellent interpersonal and analytical skills, with a solution-focused approach.
- Strong written communication and report-writing abilities.
- Ability to manage competing priorities within tight timescales.
- Experience in hospital discharge planning or community-based social work (desirable).
- Commitment to professional development and reflective supervision.
