Social Worker – Adults Social Care (Learning Disabilities)

Brief Job Overview

 
We are seeking a skilled and compassionate Social Worker to join the Adults Social Care Team at Darlington Borough Council. This is a rewarding opportunity to work with adults who have learning disabilities, helping them to live as independently and safely as possible while ensuring their wellbeing and quality of life are at the centre of every decision.  

You will work as part of a dedicated, multi-disciplinary team providing assessment, care planning, and safeguarding support in line with statutory responsibilities and best practice.

Key Responsibilities

 

  • Undertake Care Act 2014 assessments to identify the needs, strengths, and desired outcomes of adults with learning disabilities.
  • Develop and implement person-centred care and support plans that promote independence, inclusion, and wellbeing.
  • Work collaboratively with individuals, families, carers, and partner agencies to deliver high-quality and coordinated support.
  • Manage a caseload of adults with learning disabilities, ensuring all work meets statutory and local authority requirements.
  • Undertake Safeguarding Adults enquiries under Section 42 of the Care Act, ensuring timely and proportionate responses to risk.
  • Carry out reviews of existing care and support plans to ensure they remain effective and relevant to individuals’ changing needs.
  • Apply the principles of the Mental Capacity Act 2005 and ensure individuals’ rights are upheld in all decision-making.
  • Contribute to multi-agency meetings, case discussions, and best practice forums to ensure joined-up working and effective information sharing.
  • Maintain accurate, high-quality records and documentation in line with local policies and professional standards.
  • Promote a strengths-based approach to social work, empowering individuals to achieve their personal goals and outcomes.

About You

 
You are an experienced social worker with a passion for supporting adults with learning disabilities. You have strong assessment and communication skills, a person-centred approach to care, and a commitment to promoting independence and dignity. You work effectively within a multi-disciplinary setting and are confident in managing complex cases with empathy and professionalism.

Requirements

 

  • Qualified Social Worker registered with Social Work England.
  • Significant experience working with adults who have learning disabilities.
  • Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults legislation.
  • Ability to manage a varied caseload and prioritise effectively.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Experience working collaboratively within a multi-agency framework.
  • Commitment to continuous professional development and reflective practice.
  • Proficient in using electronic case management systems and maintaining accurate records.
