Community Social Worker – Mental Health

Brief Job Overview

 
We are seeking an experienced and compassionate Community Social Worker to join the Community Mental Health Service at the London Borough of Redbridge. This locum role is ideal for a skilled practitioner with experience in community-based mental health work, particularly as a Care Coordinator or within similar roles supporting adults with mental health needs.  

You will play a key role in supporting individuals in the community to maintain independence, wellbeing, and recovery through person-centred, strengths-based interventions and collaborative working with health and social care partners.

Key Responsibilities

 

  • Work as part of a multidisciplinary community mental health team to deliver high-quality, outcome-focused social work interventions.
  • Act as a Care Coordinator under the Care Programme Approach (CPA), managing a caseload of adults with complex mental health needs.
  • Undertake holistic assessments and develop personalised support and care plans that promote recovery, independence, and wellbeing.
  • Provide safeguarding interventions in line with the Care Act 2014 and local procedures, ensuring adults at risk are protected and supported.
  • Collaborate with health professionals, housing providers, and voluntary organisations to ensure a coordinated approach to care.
  • Promote social inclusion, empowerment, and access to community resources for service users and their carers.
  • Maintain accurate and timely case records, ensuring compliance with statutory duties and service standards.
  • Contribute to service improvement and best practice initiatives within the community mental health service.

About You

 
You are a confident and empathetic social work professional with proven experience in community mental health services. You are passionate about supporting adults to live independently and manage their mental health effectively. You are skilled in assessment, care coordination, and partnership working, with a strong understanding of mental health legislation and social care frameworks.

Requirements

 

  • Qualified Social Worker registered with Social Work England.
  • Experience working in a Community Mental Health Service as a Care Coordinator or in a similar role.
  • Good understanding of the Care Act 2014, Mental Health Act 1983, and related legislation and guidance.
  • Ability to manage complex caseloads and make sound professional judgements under pressure.
  • Strong partnership working skills, with the ability to work collaboratively across disciplines and agencies.
  • Excellent communication, recording, and organisational skills.
  • Commitment to person-centred, strengths-based practice and recovery-focused approaches.
