Carry out statutory duties on behalf of Hull City Council, undertaking Care Act 2014 assessments and reviews to support adults with eligible care and support needs.
Manage a caseload of adults with a range of needs, including older people, individuals with physical or learning disabilities, and mental health conditions.
Develop person-centred support plans that promote independence, wellbeing, and safety, in line with legislation and Hull City Council’s policies.
Undertake safeguarding enquiries under Section 42 of the Care Act, working with partner agencies to assess risk and implement protection plans where necessary.
Complete Mental Capacity Act assessments and contribute to best interest decisions, advocating for the rights and wishes of individuals lacking capacity.
Maintain accurate and timely case records and reports, participating in supervision, training, and development to support high standards in adult social care practice.
