Deliver statutory adult social care duties on behalf of Portsmouth City Council, completing Care Act 2014 assessments to identify eligible needs and promote independence.
Manage a diverse caseload, including older adults, individuals with physical or learning disabilities, and those with mental health needs, ensuring timely and person-centred support.
Develop and review care and support plans in collaboration with service users and carers, using a strengths-based, outcome-focused approach.
Undertake safeguarding enquiries under Section 42 of the Care Act, coordinating multi-agency responses to protect adults at risk of harm or neglect.
Complete mental capacity assessments and contribute to best interest decisions, working closely with health professionals and legal teams where required.
Maintain accurate case records and participate in regular supervision and training to ensure high standards of practice and compliance within Portsmouth City Council’s Adult Social Care service.
