Deliver statutory responsibilities for Bradford Metropolitan District Council, undertaking Care Act 2014 assessments to identify eligible needs and promote independence and wellbeing.
Manage a varied caseload, working with older adults, individuals with physical or learning disabilities, and those with mental health needs, ensuring timely and appropriate support.
Develop and review person-centred care and support plans in collaboration with service users, carers, and professionals, ensuring a strengths-based and outcomes-focused approach.
Undertake safeguarding enquiries under Section 42 of the Care Act, coordinating multi-agency responses to protect adults at risk of harm or neglect.
Complete mental capacity assessments, contribute to best interest decisions, and liaise with health professionals around Continuing Health Care (CHC) and hospital discharge processes.
Maintain accurate records and documentation in line with legal and professional standards, while participating in supervision and ongoing professional development.
