Join Bournemouth Christchurch and Poole Council to conduct statutory assessments for adults, ensuring compliance with the Care Act 2014 and other relevant legislation.
Develop, implement, and review person-centred care and support plans to promote independence, wellbeing, and safeguarding.
Manage a varied and complex caseload, prioritising high-risk and time-sensitive cases while meeting all statutory duties.
Identify and respond to safeguarding concerns, coordinating safeguarding enquiries and working closely with multi-agency partners.
Collaborate with professionals across health, housing, mental health, and voluntary sectors to deliver integrated, effective support.
