Knowsley Council is seeking a compassionate and experienced Locum Qualified Social Worker to join their Older People’s Community team. The role focuses on supporting older adults to maintain independence, wellbeing, and safety within the community through holistic assessments and coordinated care planning.
Key Responsibilities:
Conduct assessments under the Care Act to identify care and support needs of older people living in the community.
Carry out Mental Capacity Act assessments and contribute to Best Interest decisions.
Develop and review personalised care and support plans that promote independence and quality of life.
Lead safeguarding enquiries and work closely with partner agencies to protect vulnerable older adults.
Liaise with health, voluntary sector, carers, and families to ensure coordinated service delivery.
Maintain accurate and up-to-date case records using the council’s electronic system.
Requirements: Social Work England (SWE) registration, right to work in the UK, and minimum 2 years’ experience in adult social care, preferably with older people.
