Southwark Council is seeking an experienced and proactive Adults Social Worker to join our Hospital Discharge Team. This vital role supports the safe and timely discharge of adults from hospital settings into appropriate care in the community. If you’re passionate about person-centred care, reducing hospital delays, and improving outcomes for adults, we’d love to hear from you.
Key Responsibilities:
Undertake assessments under the Care Act 2014 to plan effective discharge pathways from hospital to home or care settings.
Work closely with NHS staff, therapists, families, and care providers to facilitate timely and safe discharges.
Apply strength-based approaches to promote independence and reduce long-term care needs.
Ensure legal and safeguarding responsibilities are met, including Mental Capacity Act and Deprivation of Liberty Safeguards (DoLS) assessments.
Maintain accurate records and contribute to multi-disciplinary team discussions and case conferences.
