This role is essential in supporting and safeguarding the welfare of children and young people in care. The successful candidate will work collaboratively with various stakeholders to ensure the safety, well-being, and development of children in the LAC system.
Key Responsibilities:
Conduct Assessments: Perform comprehensive assessments of children in care to identify their needs and develop effective care plans
Case Management: Manage a caseload of LAC cases, ensuring timely interventions and ongoing support
Multi-Agency Collaboration: Work closely with health professionals, educators, foster carers, and other agencies to coordinate efforts and share relevant information
Advocacy: Advocate for the rights and best interests of children in care, ensuring their voices are heard and respected in all processes
Documentation: Maintain accurate and detailed records of all activities, ensuring compliance with legal and regulatory requirements
