Job Title: Reablement Officer

Job Details:
Reablement Officer required to start ASAP
Deliver short-term reablement interventions to adults to maximise independence at home
Support individuals to regain skills and confidence, reducing or preventing ongoing care needs
Work as part of an integrated intermediate care/reablement multidisciplinary team
Contribute to person-centred care and support planning documentation
Maintain accurate and timely records in line with service and statutory requirements
Car driver preferred for community visits
Current hybrid working model – 2–3 days per week for visits, remainder home-based as appropriate
Essential Skills/Experience:
Experience in Adult Social Care Reablement and/or Rehabilitation
Experience working effectively within a Multidisciplinary Team (MDT)
Strong communication skills and ability to work collaboratively across professional boundaries
Experience contributing to clear, accurate, person-centred care and support plans
Confident using digital systems (health and/or social care platforms desirable)
