Reablement Assistant (Locum)
Location: Liverpool City Council – Granby Hub
Area: Merseyside Social Services
Rate: £19.44 per hour (umbrella, negotiable)

About the Role
An experienced Reablement Assistant is required to join the Intermediate Care and Reablement service at the Granby Hub — a 30-bed unit supporting individuals to regain and retain independence. You will work collaboratively with families, carers, and partner agencies to deliver a high-quality, coordinated reablement service.
Main Responsibilities
- Meet the physical, social and emotional needs of service users as outlined in their care plans, promoting independence and daily living skills.
- Assist service users with tasks detailed in support plans.
- Support individuals to maintain and develop daily living skills, using equipment as required.
- Liaise with multidisciplinary professionals to minimise risk and enable individuals to remain safely at home.
- Encourage development of practical skills for assisted or independent living (including household tasks, kitchen skills and managing personal health).
- Work collaboratively within an integrated health and social care team.
- Administer medication where required (for community-based staff).
- Implement personalised support plans and contribute to assessments and reviews.
- Promote dignity, empowerment and person-centred practice, respecting cultural, spiritual and religious needs.
- Hold keys to service users’ homes when required, ensuring security and confidentiality.
- Maintain accurate records and report any significant changes to the Line Manager.
- Attend supervision sessions and team training as arranged.
- Support service users in recreational, therapeutic and social activities, including hydrotherapy and rebound therapy.
- Adapt to the changing needs of the service and support delivery across different locations within the city.
- Work in accordance with Council policies, including safeguarding, equality and anti-discriminatory practice.
- Demonstrate responsible use of Council resources, reducing waste and promoting sustainability.
Skills & Experience
- NVQ Level 3 in Health and Social Care, or willingness to work towards qualification.
- Manual Handling Certificate.
- Experience supporting adults in a social care environment.
- Experience promoting independence and delivering person-centred care.
- Ability to follow structured reablement programmes.
- Strong verbal and written communication skills.
Working Hours
- Flexible rota between 08:00 and 22:00, 7 days per week.
