Community Care Support Coordinator

Brief Job Overview / Introduction to Role

We are recruiting a Community Care Support Coordinator to join the Reablement Service in the London Borough of Kingston. This ongoing locum role supports a service focused on helping individuals regain independence in their own homes. Unlike standard home care, the Reablement Service works intensively for a short period to actively assist service users in achieving personal goals.

Key Responsibilities

– Support the Reablement Manager in delivering a high-quality Reablement service. – Work with service users and carers to set goals and conduct regular goal reviews. – Carry out Reablement tasks to support service users’ independence. – Manage and record service user progress accurately. – Assist the Reablement Manager or Deputy Reablement Manager in the operational running of the service when required.

About You

– Committed to promoting independence for service users. – Excellent communication and interpersonal skills to engage with service users, carers, and colleagues. – Organised and able to maintain accurate records and reporting. – Experience in social care or a Reablement setting is advantageous.

Requirements

– Experience working in care or Reablement services is preferred. – Ability to work collaboratively within a multidisciplinary team. – Understanding of goal-setting and monitoring progress for service users. – Flexibility to work across the London Borough of Kingston as required.

Contract & Salary

– Location: London Borough of Kingston, South West London – Salary: £20.79 per hour (negotiable) – Job Type: Locum, ongoing contract
