Reablement Enabler (Part Time)

Brief Job Overview / Introduction to Role

We are recruiting a Part-Time Reablement Enabler to join the Reablement Service in the London Borough of Islington. This locum role is ongoing and focuses on supporting people in their own homes to maintain and regain independence through personalised care and Reablement plans.

Key Responsibilities

– Deliver personal care and support in line with individual Reablement plans. – Assist service users with domestic tasks, exercise programmes, and daily living activities as outlined in therapy-led plans. – Work in an enabling way, promoting independence and encouraging service users to achieve their agreed outcomes. – Collaborate closely with health and social care professionals involved in care planning and delivery. – Report directly to the Reablement Senior Enabler/Coordinator and maintain accurate records of support provided.

– You are committed to promoting independence and supporting people to live safely at home. – Strong communication skills to liaise effectively with service users, carers, and colleagues. – Ability to work as part of a multidisciplinary team and follow care plans accurately. – Flexible and reliable, with the ability to manage your time effectively during working hours.

Requirements

– Experience in a Reablement, social care, or health setting is preferred. – Understanding of person-centred care and ability to support individuals to achieve goals. – Ability to follow personalised care plans and work independently. – Available to work 25 hours per week, Monday to Friday, 16.00 – 21.00.

Contract & Salary

– Location: London Borough of Islington, North London – Salary: £24.90 per hour (negotiable) – Job Type: Locum, ongoing contract – Hours: 25 hours per week, 16.00 – 21.00
