Job Overview

Tripod Partners are currently recruiting for an experienced Band 6 Physiotherapist to join the Home 1st Therapy service at Central and North West London NHS Foundation Trust, based at Bletchley Community Hospital.

This is an ongoing locum opportunity, initially for 12 weeks, covering a combination of inpatient rehabilitation and community physiotherapy. The successful Physiotherapist will provide specialist assessment and person-centred rehabilitation across community pathways, admission avoidance, early supported discharge and inpatient rehabilitation.

This role requires a car driver with a full UK driving licence and access to a car for work, as community working is an essential part of the position.

Key Responsibilities

• Complete comprehensive physiotherapy assessments across inpatient and community settings.
• Develop and deliver person-centred rehabilitation programmes based on individual needs and goals.
• Set and review individualised rehabilitation goals with patients.
• Work autonomously across the full range of clinical physiotherapy responsibilities.
• Work across Community pathways to support admission avoidance assessments.
• Support PW1 pathways to facilitate early supported discharge from acute hospitals.
• Provide physiotherapy input across inpatient rehabilitation pathways.
• Work collaboratively with the multidisciplinary team and other services within Home 1st Planned Care.
• Liaise effectively with external agencies and partner services to coordinate patient care.
• Work in the capacity of a Trusted Assessor where required.
• Contribute to the development of integrated health and care pathways.
• Maintain accurate clinical documentation and ensure interventions are appropriately recorded.

About You

We are looking for a confident and experienced Band 6 Physiotherapist with strong clinical skills across rehabilitation and community-based practice. You will be comfortable working autonomously while also contributing effectively to a multidisciplinary team.

You will have experience working with patients across rehabilitation pathways and ideally have exposure to inpatient rehabilitation, community physiotherapy, admission avoidance and early supported discharge.

The ability to work flexibly across different clinical environments is essential. You should be confident undertaking assessments, developing individualised rehabilitation goals and working as a Trusted Assessor when required.

Requirements

• Qualified Physiotherapist with relevant professional registration.
• Relevant NHS physiotherapy experience at Band 6 level or equivalent.
• Experience in inpatient rehabilitation and community physiotherapy.
• Experience undertaking physiotherapy assessments and delivering person-centred rehabilitation.
• Experience working with admission avoidance and/or early supported discharge pathways would be advantageous.
• Ability to work autonomously and manage clinical responsibilities effectively.
• Ability to work collaboratively within a multidisciplinary team.
• Experience or understanding of Trusted Assessor responsibilities.
• Strong communication and clinical reasoning skills.
• Full UK driving licence.
• Access to a car for work is essential.
• Ability to work evenings, weekends and bank holidays as part of the service rota.

Working Hours & Shift Pattern

The position is offered at 37.5 hours per week. The service operates across a flexible shift pattern, including evenings, weekends and bank holidays.

• Standard shift patterns: 9:00am–5:00pm and 11:00am–7:00pm
• Evenings: Required as part of the service rota
• Weekends: Required as part of the service rota
• Bank Holidays: 9:00am–5:00pm
• Future service development: Potential expansion to provide 8:00am–8:00pm cover
