Brief Job Overview
Tripod Partners are recruiting a Band 6 Physiotherapist for a locum role within East Riding of Yorkshire NHS. This position sits within an Intermediate Care service at a Community Rehabilitation Centre, supporting patients through rehabilitation and discharge pathways.
The Physiotherapist will manage a caseload across community rehabilitation settings and patients’ homes, delivering treatment and supporting Discharge to Assess (D2A) pathways. The role focuses on improving mobility, promoting independence, and enabling safe transitions from hospital to community care.
Key Responsibilities
- Carry out physiotherapy assessments within intermediate care and community rehabilitation settings.
- Manage a caseload across rehabilitation centres, community settings, and patient homes.
- Deliver rehabilitation programmes to improve mobility and functional independence.
- Support discharge planning, including Discharge to Assess (D2A) pathways.
- Work collaboratively with multidisciplinary teams to provide coordinated care.
- Provide advice and support to patients, families, and carers.
- Maintain accurate clinical documentation in line with NHS standards.
- Travel across sites to support service delivery.
About You
You will be a qualified Physiotherapist with experience working within
community rehabilitation or intermediate care services. You will demonstrate strong
clinical reasoning skills and the ability to manage a varied caseload independently.
You will be confident supporting discharge planning and delivering rehabilitation interventions that
promote independence and recovery within community settings.
Requirements
- Recognised qualification in Physiotherapy.
- Registration with the Health and Care Professions Council (HCPC).
- Relevant UK experience within NHS services.
- Experience in discharge planning and/or Discharge to Assess (D2A) pathways desirable.
- Experience within community rehabilitation or intermediate care settings.
- Ability to manage a caseload independently.
- Full UK driving licence and access to a vehicle (essential).
- Experience using SystmOne desirable.
