Brief Job Overview

Tripod Partners are recruiting a Community Physiotherapist (Band 5 or Band 6) for a locum role within South East London NHS. This position sits within the Transfer of Care Hub Physiotherapy Team, supporting individuals transitioning from hospital to home through rehabilitation and discharge assessments.

The Physiotherapist will undertake discharge and rehabilitation assessments during the first six weeks following hospital discharge. Working as part of an integrated health and social care team, the role focuses on promoting independence, reducing the risk of hospital readmission, and delivering holistic rehabilitation within community settings.

Key Responsibilities

• Carry out physiotherapy discharge and rehabilitation assessments following hospital discharge.
• Support individuals during the first six weeks post-discharge to promote recovery and independence.
• Deliver rehabilitation interventions tailored to individual functional goals.
• Work collaboratively with integrated health and social care teams within the Transfer of Care Hub.
• Delegate appropriate tasks to therapy assistants and monitor progress.
• Provide feedback on patient progress during weekly multidisciplinary meetings.
• Support individuals with varying levels of care needs, including single and double-handed care arrangements.
• Maintain accurate clinical documentation in line with NHS standards.

About You

You will be a qualified Physiotherapist with experience working within community or rehabilitation services. You will demonstrate strong clinical assessment skills and the ability to support individuals transitioning from hospital-based care back into the community.

You will be comfortable working within a multidisciplinary team environment and confident supporting rehabilitation goals that promote independence and reduce the risk of hospital readmission.

Requirements

• Recognised qualification in Physiotherapy.
• Registration with the Health and Care Professions Council (HCPC).
• Experience working within community rehabilitation or discharge support services.
• Ability to carry out discharge and rehabilitation assessments.
• Experience working within multidisciplinary teams.
• Strong communication and organisational skills.

Contract & Salary

• Role: Community Physiotherapist – Band 5 or 6
• Sector: South East London NHS
• Contract Type: Locum
• Contract Length: Ongoing
• Working Hours: 37.5 hours per week
• Schedule: Monday to Friday, 08:30 – 16:30
• Salary: Negotiable
