Job Details:
Band 6 Occupational Therapist or Physiotherapist required to start ASAP within an Acute Medical Unit.
Provide assessment, treatment and rehabilitation for patients with acute medical conditions.
Support discharge planning to ensure safe and timely transitions from hospital to community settings.
Work collaboratively within a multidisciplinary team to optimise patient outcomes.
Manage a varied and fast-paced caseload within an acute environment.
Maintain accurate clinical documentation in line with NHS standards.
Skills / Experience:
HCPC registered Occupational Therapist or Physiotherapist.
Experience working within an acute hospital or medical ward setting.
Experience in discharge planning and rehabilitation.
Ability to work effectively in a fast-paced environment.
Strong communication and multidisciplinary team working skills.
