•  
  • Senior Physiotherapist required to start ASAP for 3 months within the Community Therapy Team
     

  • Act as an experienced physiotherapist supporting the development of integrated community healthcare teams within an MDT
     

  • Provide high quality physiotherapy interventions across community settings including frailty, rapid response/admission avoidance, intermediate care, falls prevention and long term condition management
     

  • Manage a defined caseload using evidence-based and client-centred principles to assess, plan, implement and evaluate interventions
     

  • Carry out advanced physiotherapy assessments for patients with complex physical and psychological presentations
     

  • Provide individual and group education and treatment programmes
     

  • Assess and provide assistive devices (e.g. orthotics) and equipment
     

  • Liaise with other services and agencies as required
     

  • Provide leadership and supervision to junior staff and physiotherapy students
     

  • Deputise for Senior Physiotherapists when required
     

  • Participate in CPD and in-service training
     

  • Undertake lone working and work flexibly, including potential future 7-day service delivery
