Location: City Health Care Partnership CIC – Highlands Health Centre, Hull & East Riding

Area: East Riding of Yorkshire NHS

Salary: £negotiable

Job Type: Locum

Contract length: Ongoing

Working hours: 37.5 hours per week, Monday to Friday, 09.00 – 17.00

 

Job overview:

 

A Band 6 Physiotherapist is required to start as soon as possible within the Falls Team.
This is a community-based role covering Hull and the East Riding area, supporting patients
who have experienced falls and are at risk of further incidents.

 

Main duties and responsibilities:

 

  •  
    • Provide specialist physiotherapy assessment and intervention for patients who have experienced falls
       

    • Deliver community-based physiotherapy services aimed at reducing falls risk and promoting independence
       

    • Assess mobility, balance, strength and functional ability, and develop tailored rehabilitation programmes
       

    • Work collaboratively within the multidisciplinary Falls Team to support holistic care planning
       

    • Provide education and advice to patients and carers to support falls prevention
       

    • Maintain accurate and timely clinical records using SystmOne
       

    • Travel across Hull and East Riding to complete community visits as required

     

    Skills and experience required:

     

    •  
      • Degree or equivalent qualification in Physiotherapy
         

      • Current professional registration
         

      • Essential community physiotherapy experience
         

      • Experience working with patients who have experienced falls
         

      • Experience using SystmOne (essential)
         

      • Ability and willingness to travel across Hull and East Riding
         

      • Strong communication, clinical reasoning and organisational skills
