Brief Job Overview
The Royal Borough of Kensington and Chelsea is seeking an Independent Living Assessor to join its busy Information and Advice (Front Door) Team. This role is the first point of contact for Adult Social Care and focuses on delivering timely information, screening referrals, and promoting independence for local residents.
This is a fast-paced, short-term intervention role suited to a proactive and enthusiastic practitioner who enjoys varied front-line work.
Key Responsibilities
- Answer incoming calls and manage a wide range of adult social care enquiries.
- Screen and process Adult Social Care referrals and safeguarding concerns.
- Provide appropriate information, advice, and signposting to residents, families, and professionals.
- Accurately record case actions, assessments, and outcomes using Mosaic.
- Support Social Work and Occupational Therapy teams with arranging services, equipment, and minor adaptations.
- Promote the independence and wellbeing of residents across the borough.
About You
You will be confident working in a high-volume environment, able to make sound screening decisions, and comfortable managing a range of enquiries. Strong organisational skills and a person-centred approach are essential.
Requirements
- Experience working within Adult Social Care or a similar front-line service.
- Knowledge of referral screening and safeguarding processes.
- Experience using Mosaic or a similar case management system.
- Strong communication and customer service skills.
- Ability to prioritise workload in a fast-paced setting.
Working Pattern
- Locum contract – Ongoing
- Office-based requirement for 50% of the working week
- Full-time hours
