Hywel Dda University Health Board is seeking a Band 6 Community Physiotherapist to support services based at Withybush General Hospital in Pembrokeshire. This role focuses on managing a mixed caseload drawn from the community physiotherapy waiting list, delivering assessment and rehabilitation across a range of patient needs within the community setting.
Key Responsibilities

- Undertake comprehensive physiotherapy assessment and treatment for community-based patients.
- Manage a varied caseload including patients with differing rehabilitation needs.
- Develop and implement individualised treatment plans using sound clinical reasoning.
- Support functional recovery and independence in the community.
- Work collaboratively with multidisciplinary and community teams.
- Maintain accurate and timely clinical records in line with NHS standards.
About You
You will be an experienced Physiotherapist with a background in community rehabilitation. You will be confident working autonomously, managing your own caseload, and delivering high-quality care in patients’ homes or community settings.
Requirements

- Degree or equivalent qualification in Physiotherapy.
- Current HCPC registration.
- Experience working within a community physiotherapy setting.
- Strong assessment, communication, and time-management skills.
Contract & Working Pattern
- Degree or equivalent qualification in Physiotherapy.

- Monday to Friday
- 08:30 – 16:30
- Competitive rate – £negotiable
