Job Details

 

An experienced Physiotherapist is required to join the Reablement Service, supporting adults within the community to regain skills, confidence and independence to remain safely at home.

 

The post holder will manage a clinical caseload, providing assessment and intervention planning for clients with diverse and complex presentations. You will work as part of an inter-professional team, with access to support and guidance from a senior therapist, contributing to safe and timely discharge planning with reduced ongoing support where possible.

 

Key Responsibilities

 

  •  
    • Undertake inter-professional assessments and independently manage a caseload of routine and complex patients requiring rehabilitation and reablement.
       

    • Negotiate and agree goals with clients, motivating participation in treatment programmes.
       

    • Formulate, deliver and evaluate individual physiotherapy programmes using clinical reasoning and a range of treatment techniques.
       

    • Provide equipment, gait re-education, postural management and graduated exercise programmes as appropriate.
       

    • Communicate effectively with clients, families, GPs and other health and social care professionals.
       

    • Educate clients about their conditions, adapting communication where barriers exist.
       

    • Participate in multi-agency meetings, case conferences and reviews.
       

    • Work collaboratively within the multidisciplinary team and attend meetings and training as required.
       

    • Support service development initiatives and contribute to education and health promotion.
       

    • Supervise Therapy Assistants and support the training of staff and students.
       

    • Maintain accurate records and data collection in line with organisational and HCPC standards.

     

    Skills & Experience

     

    •  
      • Qualified Physiotherapist with HCPC registration.
         

      • Broad physiotherapy experience, including community-based work.
         

      • Ability to work autonomously within professional guidelines.
         

      • Strong communication, organisational and clinical reasoning skills.
         

      • Full UK driving licence with access to a car and appropriate business insurance.

       

      Working Hours

       

      Monday to Friday

      Office-based role with flexibility to support urgent referrals
