Role Overview
We are seeking an experienced Band 6 Community Physiotherapist to join the team at Broomfield Hospital. This role involves delivering high-quality assessment, treatment and rehabilitation to adults in the community, supporting safe discharge and promoting independence.
Key Responsibilities
Provide physiotherapy assessments and intervention in community settings.
Support patients with rehabilitation, mobility, and functional independence.
Develop and implement personalised treatment plans.
Work collaboratively with multidisciplinary teams and external services.
Contribute to safe discharge planning and reablement goals.
Maintain accurate and timely clinical documentation.
About You
HCPC-registered Physiotherapist.
Previous community or discharge pathway experience desirable.
Strong clinical reasoning and assessment skills.
Confident working independently and as part of an MDT.
Excellent communication and organisational abilities.
