About the Role

Mid Cheshire Hospitals NHS Foundation Trust is seeking a skilled Band 6 Community Occupational Therapist or Physiotherapist to support community-based rehabilitation pathways, including Pathway 2. This role is suitable for an OT, a Physiotherapist, or a clinician with blended experience across both professions. You will deliver person-centred rehabilitation to adults in their homes or community settings, supporting recovery and promoting independence.

Main Responsibilities

• Provide rehabilitation interventions within the community, with focus on Pathway 2 discharge-to-assess pathways.
• Undertake holistic functional assessments and develop individualised therapy plans.
• Work autonomously to manage a caseload across various community locations.
• Support safe discharge, recovery and independence following hospital admission.
• Collaborate with the multidisciplinary team to ensure coordinated and effective care.
• Maintain accurate and timely documentation in line with Trust and HCPC standards.

Skills & Experience Required

• HCPC-registered Occupational Therapist or Physiotherapist.
• Experience in community rehabilitation and/or Pathway 2 services.
• Strong assessment, clinical reasoning and caseload management skills.
• Ability to work independently within community environments.
• Excellent communication and MDT collaboration skills.

Working Hours

• Monday to Friday, 09:00 – 17:00.
• Full-time hours.
