UEC Therapist – Band 7

 

Job Overview

We’re seeking an experienced Occupational Therapist or Physiotherapist to join Shropshire Community Health Trust as a Band 7 UEC Therapist within the Care Transfer Hub. This locum position plays a key role in leading complex discharge planning and ensuring patients receive safe, person-centred transitions from hospital to home or community care.  

 

Working within the Urgent and Emergency Care (UEC) pathway, you will champion the “home first” approach and support integrated, multi-disciplinary working to optimise outcomes for patients across acute and community settings.

 

Key Responsibilities

– Act as a senior clinician and clinical lead within the Care Transfer Hub multidisciplinary team, promoting a seamless discharge process across hospital and community care.  

– Manage a complex caseload independently, providing specialist clinical input and decision-making in line with evidence-based practice and NICE guidance.  

– Assess, plan, and coordinate safe and timely discharges, ensuring the right pathway for each patient under the discharge-to-assess model.  

– Provide advanced clinical advice to colleagues, patients, and carers on treatment options and discharge planning.  

– Support positive risk management to promote patient safety and enable least restrictive discharge solutions.  

– Build and maintain effective relationships with care homes, community teams, and partner agencies to ensure continuity of care.  

– Supervise and guide junior members of the therapy team, contributing to service development and clinical governance.  

– Demonstrate advanced communication skills in complex or sensitive situations involving patients, families, and professionals.  

– Participate in 7-day service delivery, ensuring flexible and responsive support across acute and community sites.
