We are looking for a skilled and motivated Band 6 Physiotherapist to join Shropshire Community Health Trust. This role focuses on delivering high-quality physiotherapy services within community settings, with a particular emphasis on mobility assessments and promoting independence. The successful candidate will have strong clinical reasoning skills, experience in community rehabilitation, and the ability to work autonomously while collaborating effectively with multidisciplinary teams.
Undertake comprehensive mobility and functional assessments for adults in community settings
Develop and implement personalised physiotherapy treatment plans to promote safe mobility and reduce falls risk
Work collaboratively with service users, carers, and a wider multidisciplinary team to support holistic care
Provide clinical supervision and guidance to junior staff and students as required
Maintain accurate and timely clinical documentation in line with professional and Trust standards
Participate in service development, audits, and ongoing professional development to support best practice
