As a Physiotherapist with North East London NHS Foundation Trust, working at Band 5 within community rehabilitation and neurology services, you will provide specialist physiotherapy assessment and intervention to support patients’ recovery and independence. Collaborating closely with patients, families, carers and multidisciplinary teams, you will deliver personalised rehabilitation programmes tailored to neurological needs.
Conduct comprehensive assessments of patients’ physical and neurological function.
Develop, implement and review individualised physiotherapy intervention plans in line with best practice.
Work collaboratively with patients, carers and professionals to coordinate care and facilitate access to community resources.
Maintain accurate and timely clinical records in accordance with NHS and professional standards.
Provide education, advice and support to patients and carers on managing neurological conditions.
Contribute to service development and quality improvement initiatives within the physiotherapy team.
