Somerset NHS Foundation Trust is looking for a qualified and experienced Band 6 Physiotherapist to join the Adult Community service. This role will focus on providing specialist physiotherapy within community and reablement settings to support patient recovery, mobility, and independence.
Undertake comprehensive physiotherapy assessments and interventions for adult community patients
Deliver tailored rehabilitation and reablement programmes to support functional recovery and independence
Work collaboratively with the multidisciplinary team to develop and implement effective care plans
Advise and educate patients and families on exercises, mobility strategies, and safe daily living
Monitor patient progress and adjust treatment plans to meet individual needs
Contribute to service development initiatives and provide guidance to junior staff where required
