As a Physiotherapist with Central and North West London NHS Foundation Trust, working in community settings, you will provide physiotherapy services to adult patients with a focus on falls prevention and mobility assessments. Operating at Band 6, you will act as a Best Interest Assessor and support patients with telecare, collaborating closely with multidisciplinary teams to deliver tailored therapy that enhances patient safety and independence.
Conduct comprehensive physiotherapy assessments of adult patients in community settings.
Develop, implement and evaluate personalised physiotherapy programmes focused on falls prevention and mobility.
Work collaboratively with patients, families, carers and healthcare professionals to coordinate care and support.
Act as a Best Interest Assessor where required.
Support patients with telecare interventions.
Maintain accurate and detailed clinical records in accordance with professional and organisational standards.
