Physiotherapist – Band 6 (5 Roles)

Tripod Partners are recruiting five experienced Band 6 Physiotherapists for exciting locum opportunities with NELFT, based at Grays Court Community Hospital, providing services across Redbridge, Havering, and Barking & Dagenham. Working within the North East London NHS Intensive Rehabilitation & Therapy Service, you will deliver urgent community-based physiotherapy assessments and rehabilitation interventions that prevent unnecessary hospital admissions and support people to remain safely in their own homes.
This is an excellent opportunity for Physiotherapists with community or rapid response experience who thrive in a fast-paced multidisciplinary environment. You will assess patients experiencing acute mobility or functional decline, deliver timely rehabilitation, and work collaboratively with health and social care partners to maximise recovery, independence, and quality of life.
Key Responsibilities
- Provide urgent physiotherapy assessments and treatment to prevent unnecessary hospital admissions.
- Respond rapidly to referrals for patients experiencing acute functional or mobility decline.
- Assess mobility, balance, strength, transfers, gait, and falls risk within community settings.
- Develop and implement immediate rehabilitation and management plans to support patients safely at home.
- Deliver evidence-based rehabilitation interventions that maximise independence and reduce functional deterioration.
- Facilitate early supported discharge and help prevent avoidable hospital readmissions.
- Recommend appropriate equipment, exercise programmes, and environmental adaptations to reduce risk and promote safe mobility.
- Provide education, self-management strategies, and rehabilitation advice to patients, families, and carers.
- Work collaboratively with multidisciplinary teams to deliver coordinated crisis response and integrated community care.
- Monitor patient progress, review treatment plans, and adapt interventions to achieve the best possible outcomes.
- Maintain accurate clinical documentation and contribute to achieving service quality and performance targets.
- Participate in the delivery of a seven-day rehabilitation service across three London boroughs.
About You
You will be a HCPC-registered Physiotherapist with post-qualification NHS experience, ideally gained within community rehabilitation, rapid response, Intermediate Care, or urgent care services. You will be confident working autonomously while contributing effectively within a multidisciplinary team.
You will possess excellent assessment, rehabilitation, communication, and clinical reasoning skills, together with a proactive and patient-centred approach to supporting adults with complex rehabilitation needs in the community.
Requirements
- Degree or Diploma in Physiotherapy.
- Current HCPC registration.
- Post-qualification NHS Physiotherapy experience.
- Experience within community rehabilitation, rapid response, Intermediate Care, or urgent community services is highly desirable.
- Strong assessment and rehabilitation skills relating to mobility, falls prevention, strength, balance, and functional independence.
- Excellent communication, clinical reasoning, and multidisciplinary team-working skills.
- Full UK driving licence and access to a vehicle are essential due to community working across Redbridge, Havering, and Barking & Dagenham.
- Ability to participate in a seven-day service.
